Catholic Health in New York is seeking a compassionate Home Care Transition Nurse to guide patients from hospital to home care. This RN will assess, plan, and develop referrals, collaborating with physicians, discharge planners, families, and partners to ensure safe transitions.
The role requires 5 eight-hour shifts (9:30 AM–5:30 PM) with rotating weekends and holidays, travel to Good Samaritan Hospital, and a NY RN license with at least 2 years of home care experience.
