RN Head of Care Management

As the clinical lead for Clara's care management efforts, the full-time RN Head of Care Management will ensure adherence to clinical protocols, oversee quality across programs, and make critical operational decisions while working remotely. Key responsibilities Translate clinical protocols into Clara's conversation design and define triage logic for patient escalations Build and manage Clara's clinical quality program, tracking metrics and resolving clinical incidents Serve as the final clinical sign-off for new workflows and partner with customers during implementation and reviews Required qualifications Active, unrestricted RN license 7+ years of clinical nursing experience, including 5+ years in care management Hands-on experience with transitional care and chronic care management programs Working knowledge of quality programs (HEDIS, Stars) and CMS care management requirements Experience in building or improving care management protocols and workflows
