RN Float Case Manager – Weekend Care Coordination

Boston Medical Center is seeking an RN Float Case Manager for the Care Management department. The role involves a collaborative process to assess, plan, implement, monitor and evaluate services to meet complex health needs, using resources to obtain quality, cost-effective outcomes for patients and payers.

The position requires a BSN, Massachusetts RN licensure, and a CCM or related certification is preferred within 24 months of hire.
