About This Role

The RN, Field Care Manager (Maternal-Child Health) assesses and evaluates members' needs to achieve optimal wellness by guiding members and families toward appropriate resources. This role is ideal for experienced nurses who excel in a holistic, member-focused approach to care.

Highlights

• Pay : $71,100 - $97,800/year
• Location : Remote, LA
• Residency : Candidates must reside in Region 6, 7, or 8 of Northern Louisiana
• Work Style : Primarily field-based with a remote home office
• Schedule : Monday through Friday, 8:00 AM to 5:00 PM CT
• Travel : Up to 50% within the assigned Louisiana region

What You’ll Do

• Assess and evaluate members' needs for optimal wellness.
• Conduct telephonic and in-person assessments to identify member needs.
• Develop individualized care plans using clinical judgment.
• Collaborate with providers and community services for quality outcomes.
• Coordinate services for physical health and social determinants of health.
• Facilitate communication across the care team.
• Support optimal health through regular monitoring and evaluation.
• Identify and resolve barriers to effective care.
• Participate in health promotion and community events.
• Submit incident reports as required.

Requirements

• Education :
  • Active licensed Registered Nurse (RN) in the state of Louisiana without restrictions
• Experience :
  • Two (2) or more years of clinical experience in maternal-child health, Labor & Delivery (L&D), and/or pediatrics
• Licenses & Certifications :
  • One of the following licenses required: RN (Registered Nurse) - State Licensure required
  • Case Management Certification (CCM) preferred or willingness to obtain within two (2) years of employment
