RN, Field Care Manager

About This Role

Humana Healthy Horizons in Virginia is seeking RN, Field Care Managers to perform telephonic and face-to-face assessments for long-term services and support (LTSS) and aged, blind, and disabled (ABD) member populations. This role is ideal for experienced nurses who can work independently and make autonomous decisions while providing care and support to members.

Highlights

• **Pay:** $71,100 - $97,800/year
• **Location:** Chester, VA
• **Residency:** Must reside in the Central Region of Virginia (Richmond, Henrico, or Midlothian areas)
• **Schedule:** Monday – Friday; 8:0am – 5:00pm EST
• **Travel:** 50 - 75% field interactions with members and families

What You'll Do

• Perform telephonic and face-to-face assessments of member needs.
• Develop individualized care plans for members.
• Identify and resolve barriers to effective care.
• Monitor member progress towards desired health outcomes.
• Utilize evidence-based strategies to manage members' health.
• Make autonomous decisions in ambiguous situations.
• Guide members and families towards appropriate resources.
• Conduct comprehensive evaluations of medical complexity.
• Spend quality time assessing members' goals and needs.
• Follow established guidelines and procedures.

Requirements

Education:

• Active Registered Nurse (RN) license in Virginia required.

Experience:

• 2 years of prior experience in healthcare and/or case management.
• 1 year of experience working with individuals at risk for chronic conditions.

Licenses & Certifications:

• Active RN license in Virginia without disciplinary action required.
