- Perform telephonic and face-to-face assessments and evaluations of LTSS population needs and requirements
- Meet members in the field at their homes or locations of their choice
- Assess members’ goals, needs, and barriers
- Connect members with quality services to promote well-being and person-centered health outcomes
- Review and analyze members for medical complexity
- Develop individualized care plans
- Apply evidence-based strategies to manage physical, environmental, and psychosocial health
- Identify and resolve barriers to effective care
- Continuously monitor care through comprehensive assessments and evaluations
- Cover colleagues’ caseloads of aged, blind, and disabled members as needed
- Work with minimal direction and receive guidance where needed
- Understand and apply department, segment, and organizational strategy and operating objectives
- Follow established guidelines and procedures
- Travel 50–75% for field interactions with members, families, and providers
- Attend occasional onsite meetings at the Humana Healthy Horizons office in Glen Allen, Virginia
- Work Monday–Friday, 8:00 a.m.–5:00 p.m. Eastern Standard Time
Requirements
- Must reside in the Central Region in the Commonwealth of Virginia in Richmond, Glen Allen, Henrico, Sandston, or Chesterfield
- Active Registered Nurse (RN) license in the Commonwealth of Virginia without disciplinary action
- Two (2) years of prior experience in health care and/or case management
- One (1) year of experience working with individuals who meet the Cardinal Care Priority Population criteria
- Knowledge of community health and social service agencies and additional community resources
- Intermediate to advanced computer skills
- Experience with Microsoft Word, Outlook, and Excel
- Ability to use electronic information applications/software programs including electronic medical records
- Exceptional communication and interpersonal skills with the ability to quickly build rapport
- Ability to work a full-time schedule
- Ability to travel to region-based facilities and homes for face-to-face assessments and interactions
- Bachelor of Science in Nursing (BSN) preferred
- Certification in Case Management (CCM) preferred
- Previous managed care experience preferred
- Motivational Interviewing Certification and/or knowledge preferred
- 3–5 years of in-home assessment and care coordination experience preferred
- Experience with disease management, health promotion, coaching and wellness preferred
- Bilingual or multilingual abilities in English/Spanish, Arabic, Vietnamese, Amharic, Urdu, or another language preferred; must speak, read, and write both languages without limitations and assistance
- Must complete an ILR language proficiency assessment when speaking with members in a language other than English
- Must undergo tuberculosis (TB) screening if selected
- Self-provided internet service must provide at least 25 Mbps download and 10 Mbps upload for home or hybrid home/office work
- Must work from a dedicated space lacking ongoing interruptions to protect member PHI/HIPAA information
