Providing member-centric care in a remote setting, the full-time RN Complex Case Manager will manage a portfolio of high-need clients, coordinating services and resources to ensure safe transitions within the healthcare system. Key responsibilities Conduct individualized assessments to identify members' goals of care and coordinate necessary services Support members in navigating the complexities of the healthcare system and connect them with community resources Collaborate with healthcare providers to ensure effective communication and continuity of care Required qualifications Associate's or Bachelor's degree in Nursing 5+ years of clinical/acute care experience beyond degree Current, unrestricted RN license in the state of residence Certified Case Manager (CCM) preferred, or commitment to obtain within two years of hire Experience working with vulnerable and complex populations in a clinical or telephonic environment
