RN, Community-Based Care Management & Coordination

Hackensack Meridian Health is hiring an RN for Community Based Care Management to support physician practices and identified complex patients. The role focuses on coordinated care, education, supervision, and resource navigation across the care continuum.

The RN will work in ambulatory and hospital settings, review gaps in care, engage patients and caregivers, and collaborate with PCPs to improve quality and reduce costs. Minimum 3–5 years of relevant experience is required.
