RN Case Manager
8a - 4p; Rotating weekends every 4th or 5th
Must have exp. with Sub-acute patients
3 month assignment, possible extension
Responsibilities
- Coordinates patients through the acute care hospitalization
- Manages all aspects of discharge planning/transition process
- Performs utilization management and quality screenings
- In-Patient Case Management is required
Qualifications
- 2 years of acute care experience strongly preferred
- In-Patient Case Management is required
- Must have exp. with Sub- acute patients
- BSN required
- RN license required
- CCMC preferred
