POSITION SUMMARY – RN Case Manager

Coordinates comprehensive care for patients across the continuum, conducting assessments, developing care plans, advocating for patient needs, facilitating transitions between care settings, collaborating with interdisciplinary teams and community resources, monitoring outcomes, and promoting effective utilization of clinical and support services to improve patient care and access.

MINIMUM REQUIRED QUALIFICATIONS

• VT/compact RN license
• AHA BLS
• BSN
• 3 years clinical experience
• Recent acute care hospital experience

PREFERRED QUALIFICATIONS

• Previous travel
• Utilization review experience

LENGTH OF ASSIGNMENT – 1/2/27 End Date (14 weeks)

SHIFT / HOURS PER WEEK – 8:00 AM - 4:30 PM. Call TBD.
