The RN Case Manager is responsible for ensuring high-value patient care that is coordinated and efficient, collaborating with the healthcare team to achieve specific patient outcomes and optimize resource use.

What You’ll Do
- Coordinate and implement the plan of care as part of the interdisciplinary health care team.
- Establish a system for linking inpatient care with outpatient services and case management.
- Optimize hospital systems impacting quality and length of stay.
- Ensure patients meet clinical needs for admission, treatment, and discharge.
- Assist clinicians in documenting appropriateness of admissions and continued stays.
- Develop and implement appropriate discharge plans with healthcare team members.
- Contact outside agencies and initiate necessary referrals.
- Advocate for patients and families, linking them with appropriate resources.
Requirements
- Education: Minimum of a Baccalaureate degree in clinically related field. R.N. required.
- Experience: Minimum of three (3) years of relevant clinical experience.
- Licenses & Certifications: RN Licensure in Rhode Island required. Additional RN licensure in CT preferred.
