RN, Case Manager UM

JOB SUMMARYThis position is for a Case Manager whose responsibilities includes assessing treatment needs; developing, monitoring, and evaluating treatment plans and progress; facilitating inter-disciplinary care activities; assists with Swing Bed Program: and coordinates the continued care of transitioned patients. The position reports to the Chief Nursing Officer.JOB QUALIFICATIONSMinimum level of Education: Education level equivalent to completion of an associate degree in nursing.Formal Training: Clinical competence in nursing preferred.Licensure, Certifications & Registration: Current license to practice professional nursing in the state of Georgia Current license to practice professional nursing in the state of Georgia; BLS Provider certification required. Instructor certifications do not replace provider certification requirements.Work Experience: Basic technical nursing skills. Experience in chart auditing. Experience in interpreting federal and state regulations and accreditation standards governing patient medical necessity and continuity. Basic typing and computer skills.
