The RN Case Manager in Transplant at Mass General Brigham is responsible for enhancing the delivery of patient care services along the continuum of care. This role is ideal for dedicated nurses who are focused on improving patient experiences and ensuring high-quality care.

What You'll Do
- Collaborate with the healthcare team to enhance patient care services.
- Continuously improve the patient experience and ensure institutional standards.
- Evaluate, predict, and facilitate the trajectory of patient care.
- Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
- Participate in an in-depth orientation and onboarding program.
- Engage in the "Novice to Expert" program for professional development.
- Utilize resources from strong leaders and case management specialists.
Requirements
- Education: ADN required, BSN strongly preferred
- Experience: Minimum of 2 years Med/Surg experience required
- RN Case Manager experience required
- Acute hospital experience preferred
- Licenses & Certifications: Current RN License in Massachusetts
- Case Manager certificate preferred
