RN Case Manager: Transitional Care Navigator

Cleveland Clinic’s Martin North Hospital is seeking an RN Case Manager to collaborate with inter-professional teams, coordinate transitions of care, and ensure continuity of care for patients across the hospital system.

Role focuses on advocacy, holistic assessment and impactful care planning, with duties including patient-centered rounds and interdepartmental collaboration. Day shifts 8:00AM-4:30PM.
