Role:

Discharge Planning Nurse

Responsibilities:

Assess, develop, implement, and evaluate patients' discharge plans in collaboration with an interdisciplinary team to ensure smooth transition to the next level of care.

  • Ensure compliance with discharge planning regulations and identify gaps in inpatient and observation stays.
  • Assess high-risk patients for readmission, identify root causes, and collaborate on discharge strategies.
  • Participate in quality improvement initiatives and may handle utilization management, ED case management, and act as a liaison with post-acute providers.
  • Apply evidence-based practices, adhere to professional nursing standards, and follow the ANA Code of Ethics.
  • Maintain safety awareness and perform additional duties as assigned.

Qualifications & Skills:

Graduate of an accredited nursing program, with at least three years of acute care experience.

Licenses:

Valid RN license in applicable states, including Illinois, Missouri, Oklahoma, or Wisconsin, with appropriate certifications.

Physical Requirements:

Ability to perform frequent lifting, sitting, standing, walking, and other physical activities, along with good vision and hearing capabilities.

Work Schedule:

Day shift, 8:00 am - 4:30 pm, with a 40-hour workweek.
