Home health experience ideal, and we will provide training and support. Revise as needed in consultation with the patient, physician, and other team members when appropriate.Establish patient-specific goals and incorporate therapeutic, preventative and rehabilitative nursing services.Responsible for the oversight and delivery of quality, patient-centered care rendered in the patient's home or living facility.Communicate necessary revisions to patient status and/or needs with the entire care teamRegularly consult, advise and reevaluate patient status with patient, family, patient representative and other team members as appropriate regarding safety, plan and outcome of care and discharge planning.Provide complete, accurate and timely documentation consistent with the plan of care
