Description

Join Our Team as an RN Case Manager for home visits in beautiful downtown Holland! Atrio Home Care, the partnership of Holland Home and Resthaven, is seeking a compassionate RN Case Manager to join our Holland / Lakeshoreteam. This position will involve providing direct care to clients in Ottawa and Allegan Counties.

Responsibilities

• Perform initial and comprehensive assessments and deliver compassionate care to clients
• Collaborate with physicians and other care team members to develop a Plan of Care
• Promotes client and family/caregiver independence through counseling and education
• Identify barriers to desired outcomes and evaluates outcomes of care
• Prepare clinical documentation, progress notes in a timely manner using iPads for theEMR system
• Participates in on‑call rotation

Schedule/Shift

• Monday - Friday (full or part‑time opportunities available)
• Day shift 8-5 pm, with flexibility in scheduling to help you maintain a healthy work/life balance

Qualifications

• Licensed RN in Michigan; BSN preferred
• Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits
• 1-2 years of acute care or skilled nursing experience; prior certified home health experience preferred
• Passion for delivering high quality care to older adults
