Atrio Home Care, the partnership of Holland Home and Resthaven, is seeking a compassionate RN Case Manager to join our Holland / Lakeshore team. This position will involve providing direct care to clients in Ottawa and Allegan Counties.

Brief Description

Join Our Team as an RN Case Manager for home visits in beautiful downtown Holland! Atrio Home Care, the partnership of Holland Home and Resthaven, is seeking a compassionate RN Case Manager to join our Holland / Lakeshore team. This position will involve providing direct care to clients in Ottawa and Allegan Counties.

Responsibilities

• Perform initial and comprehensive assessments and deliver compassionate care to clients
• Collaborate with physicians and other care team members to develop a Plan of Care
• Promotes client and family/caregiver independence through counseling and education
• Coordinate client discharge planning
• Identify barriers to desired outcomes and evaluates outcomes of care
• Prepare clinical documentation, progress notes in a timely manner using iPads for the EMR system
• Participates in on-call rotation

Schedule/Shift

• Monday - Friday (full or part-time opportunities available)
• Day shift 8-5 pm, with flexibility in scheduling to help you maintain a healthy work/life balance

Qualifications

• Licensed RN in Michigan; BSN preferred
• Valid Michigan driver's license, proof of auto insurance, and reliable transportation for home visits
• 1-2 years of acute care or skilled nursing experience; prior certified home health experience preferred
• Passion for delivering high quality care to older adults
