RN Case Manager

To facilitate high-quality kidney health management, the fully remote RN Case Manager will manage a caseload of mixed acuity members, conduct health screenings, create care plans, and collaborate with healthcare professionals to ensure effective patient care. Key responsibilities Handle inbound and outbound calls to provide exceptional service and education regarding kidney health and treatment options Engage members in HealthMap's Kidney Health Program and follow up based on policy guidelines Coordinate care among patients, their support networks, and healthcare providers to resolve barriers and assist in meeting individual health goals Required qualifications Active, unrestricted RN license required Bachelor's degree required; or 6+ years of RN experience including 3+ years in case management may be considered in lieu of degree CCM certification preferred Three (3) years of experience in case management preferred Experience in a dialysis center or transplant center preferred
