RN Case Manager for Vitality Living of Remer!!
Vitality Living is seeking a results-driven RN Case Manager to support Vitality Living of Park Rapids in a fast-paced, execution-focused environment. This role is ideal for a nurse who can move quickly, lead confidently, and ensure high-quality, compliant care.
Schedule: 30 hours/week
No over night call - after-hours supported by ACS
No weekends and no holidays!
Key Responsibilities:
- Complete nursing assessments (initial, routine, change-in-condition)
- Develop and manage resident care plans
- Ensure accurate, compliant documentation (144G)
- Provide clinical leadership and support to care teams
- Collaborate to drive quality outcomes and resident safety
- Care Coordination with providers & Families
Qualifications:
- Active Minnesota RN license (required)
- 3-5 years Assisted Living Experience or equivalent (required)
- Strong knowledge of assessments, care planning, and compliance
- Ability to execute quickly and work independently
