As a Case Manager RN, you coordinate rehabilitation and acute care services to support safe, patient-centered care transitions. You guide patients and families through program orientation, benefits, and rights; complete psychosocial assessments; and lead discharge planning. Working closely with physicians, nursing, and case management leadership, you develop individualized care plans, arrange resources, and promote education that aligns treatment goals with each patient’s needs.
