Role Overview
This position provides PRN care management support, focusing on advocating for patients’ health needs, addressing barriers to care, and developing interdisciplinary care plans.
Key Responsibilities
- Assess and plan for patients with complex medical conditions, ensuring safe transitions between care levels.
- Utilize telehealth, home, or field-based settings to deliver care and collaborate with interdisciplinary teams.
- Engage patients and families to identify goals, support functional independence, and address care gaps.
- Leverage community resources, ensure legal and ethical compliance, and document activities per standards.
- Work independently within scope, demonstrating strong communication, assessment, and problem-solving skills.
Qualifications & Skills
- Current RN license with a BSN or higher degree.
- Preferred CCM or RN-BC certification, attainable within 36 months.
- Excellent interpersonal, verbal, and written communication skills.
- Proficiency in Microsoft Office and understanding of managed care principles.
- Ability to manage multiple tasks in a fast-paced, evolving environment with critical thinking skills.
