RN Case Manager Per Diem, Weekend Rotations (LA)

UCLA Health in Los Angeles is seeking an experienced RN Case Manager for per diem shifts, weekends on a rotating basis, with 10-hour days 7:00am-5:30pm and holiday coverage. This role focuses on assessing and coordinating care across a diverse patient population.

You will collaborate with a multidisciplinary team, implement individualized care plans using risk stratification, and conduct utilization review to ensure safe, high-quality, cost-efficient care.
