The RN Case Manager, Per Diem at Hackensack Meridian Health is responsible for coordinating patient care activities to ensure compliance with evidence-based practice standards and regulatory requirements. What You'll Do
- Coordinate patient care progression throughout the continuum.
- Facilitate collaborative management of patient care, intervening to remove barriers.
- Apply process improvement methodologies to evaluate care outcomes.
- Communicate with physicians, nursing, and ancillary staff.
- Evaluate care against Length of Stay metrics.
- Collaborate with Admitting for accurate patient demographic and insurance information.
- Communicate medical plan of care and discharge plan to patients and families.
- Assess and plan for discharge needs and arrangements.
- Complete utilization management for assigned patients.
- Monitor length of stay and clinical resource use.
Requirements
Education
- Bachelor's degree in nursing (BSN) preferred; 1 year experience for non-BSN HackensackUMC employees.
Experience
- 2+ years clinical experience in relevant specialty preferred.
- Minimum of 2 years in Case Management in an acute care setting preferred.
Licenses & Certifications
- Current NJ State Professional Registered Nurse License required.
- BLS AHA Basic Health Care Life Support HCP Certification required.
