RN Case Manager - Part-Time

About This Role

The RN Case Manager at Marymount Hospital works collaboratively with inter-professional teams to facilitate effective transitional care coordination. This part-time role is ideal for experienced nurses who are passionate about patient advocacy and holistic care.

What You'll Do

• Ensure compliance with standards of accrediting bodies and state mandates.
• Promote interprofessional synchronization throughout the acute episode.
• Complete effective handoffs to the next level of care or community.
• Proactively assess and mitigate risks to patients’ transitions of care.
• Work with patients and care partners to develop individualized transitional care plans.
• Participate and lead in patient rounding activities, huddles, and committee meetings.

Requirements

Education:

• Graduate from an accredited school of professional nursing.
• BSN preferred.

Experience:

• 3 years of recent RN clinical experience.
• 2 years of recent experience in utilization review, quality, or care management preferred.

Licenses & Certifications:

• Current RN License in Ohio.
• Basic Life Support (BLS) certification required.
• Certification by an approved entity preferred within one year of eligibility (CTM, ACM, CCM, or AHA).
