RN - Case Manager

Description:Coordinates management of care and ensures optimum utilization of resources.Ensures service delivery and compliance with external review agencies.Provides ongoing support and expertise through comprehensive assessment.Develops care plans and implements strategies based on individual patient needs.Evaluates overall patient needs to enhance quality of management and satisfaction.Promotes continuity of care and cost-effectiveness through integration of functions.Engages in case management, utilization review, management, and discharge planning.Serves as a patient advocate by addressing holistic needs during resource utilization.Supports effective discharge planning through patient-centered approaches.
