What You'll Do
- Collaborate with the health care team to enhance patient care services along the continuum of care.
- Meet patient needs efficiently and expeditiously.
- Continuously improve the patient experience.
- Ensure institutional standards of high-quality patient care.
- Reduce costs and ensure reimbursement.
- Evaluate, predict, and facilitate the trajectory of patient care.
- Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
Requirements
Education
Graduate of an accredited nursing program.
Experience
Previous experience in case management preferred.
Licenses & Certifications
Registered Nurse (RN - State License) required.
