Provide compassionate hospice care in patients’ homes or care facilities. Perform evaluations, assessments, and treatments in accordance with policy and individualized care plans. Maintain accurate documentation and records. Follow established policies and procedures within scope of practice. Ensure continuity of patient care.

Qualifications
- Current, active, and unrestricted Registered Nurse (RN) licensure in the state of practice
