What You'll Do
- Assess individual health care needs across the continuum.
- Plan and coordinate appropriate levels of care and post-acute care needs.
- Monitor and evaluate options and services for patients.
- Collaborate with physicians and staff to determine care requirements.
Requirements
Education:- Other Certificate/Diploma in Nursing required, Bachelor's Degree in Nursing preferred
- 1-2 years of related clinical, continuing care, or case management experience preferred
- Current Registered Nurse (RN) License in Massachusetts required
- Case Management certification or active pursuit of certification preferred
