POSITION SUMMARY
Case Manager - Typically placed on Med-Surg or Tele floors following Internal Medicine and Family Medicine.
RESPONSIBILITIES
- Knowledge of discharge planning and making referrals.
- Typically work on a Medical-Surgical (MS) or Tele unit and split the floor with a Social Worker.
- function independently on the unit and collaborate with an interdisciplinary team and patients for discharge planning.
- Round on the unit with physicians and the team.
- Maintain a positive attitude, think critically, and have strong time management skills.
- Utilization Review (UR) is a separate department responsible for insurance approvals and arranging transportation.
MINIMUM QUALIFICATIONS
- 1 year of minimum experience.
- BLS Certification.
- PRI (Patient Review Instrument) Certification.
- SCREEN Training Certification.
ADDITIONAL CERTIFICATIONS
PRI & SCREEN certifications can be taken online through the School of Albany; typically take a couple of hours for each certification. Cost is approximately $300 per certification. Reference:
