What You'll Do
- Coordinate and support healthcare within the facility.
- Manage referrals for services outside the clinic.
- Provide outreach and enrollment services to meet eligibility requirements.
- Ensure coordination and planning, including community and family support.
- Develop and coordinate community service plans.
- Partner with service providers, families, and patients to create healthcare plans.
- Modify patient treatment plans based on responses and conditions.
- Prepare patients and families for discharge.
- Communicate with third-party payers for service authorizations.
- Maintain accurate, detailed reports and records.
Requirements
Education
- Bachelor's Degree in Nursing preferred
Experience
- 2-3 years in case management, utilization review, or discharge planning preferred
Licenses & Certifications
- Current MA RN License required
- BLS preferred
