Description:
Start date: ASAP
Floating expectation required.Ratio: 1:25.Job involves coordinating care for patients and managing their discharge plans.Candidates will participate in the development of treatment plans.Experience with arranging durable medical equipment, skilled nursing facilities, and ambulance transport is essential.Candidates must be competent to perform these tasks independently.The role requires proficiency in computerized documentation systems.Charting experience in an electronic system is a hard requirement.Candidates should have knowledge of various regulatory requirements related to healthcare services.Weekends and days off will vary based on scheduling needs.Consecutive days off are not guaranteed but will be accommodated when possible.Strong organizational skills and the ability to work in a fast-paced environment are necessary for success in this role.Candidates must commit to working through holidays as required by the department.Travelers may be expected to float between nearby facilities as needed.Refusal to float may result in termination of assignment.
