What You'll Do

  • Collaborate with the healthcare team to enhance patient care services.
  • Evaluate, predict, and facilitate the trajectory of patient care.
  • Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
  • Continuously improve the patient experience while ensuring high-quality care.
  • Participate in the Case Management department's orientation and onboarding program.
  • Engage in the "Novice to Expert" program for professional development.
  • Utilize resources from case management specialists and strong leadership support.

Requirements

Education

  • ADN required, BSN strongly preferred

Experience

  • Minimum of 2 years Med/Surg experience required
  • RN Case Manager experience requiredAcute hospital experience preferred

Licenses & Certifications

  • Must be a licensed RN in the state of Massachusetts
  • Case Manager certificate preferred
