Crossbridge Hospice, LLC is seeking a compassionate RN Case Manager to deliver end-of-life care in patients’ homes or community settings. You will oversee intermittent skilled nursing services, supervise hospice aides, and guide families through meaningful, highly sensitive care experiences.
Candidates should have an active RN license, a valid driver's license, and at least two years of RN experience with Hospice/Palliative nursing preferred.
