Description: The RN Case Manager will work as part of a multidisciplinary team.The role involves supporting patients with liver failure as they enter the hospital, including emergency department, observation, and inpatient settings.The RN Case Manager will navigate care and discharge planning effectively.This position does not require weekend shifts.First-time travelers are welcome, provided all experience requirements are met.Candidates must have experience with care coordination and discharge planning.Charting is done using Epic.Business casual dress attire is required; no scrubs are needed.The daily patient census is approximately 25, focusing on adult and geriatric populations.Excellent communication and collaboration skills are essential for this role.The RN Case Manager will utilize InterQual criteria and comply with regulations from various health oversight agencies.
