Care Coordination / Continuum of Care / Discharge Planning / Patient Focus and Care Plans

MINIMUM REQUIRED QUALIFICATIONS

CA RN license; BLS; LA City Fire & Safety; 2 years recent acute hospital care coordination experience; Prior CommonSpirit/Dignity experience
