HealthONE is seeking an RN Case Manager to coordinate patient care across our Denver facilities. In this role, you’ll perform clinical assessments, develop and implement individualized care plans, and manage safe transitions of care from admission through discharge. You will collaborate closely with physicians, nurses, social workers, and community partners to optimize length of stay, ensure appropriate resource utilization, and support patients and families with education and advocacy.
