As an RN Case Manager at Moments Hospice, you will play a critical role in making our patients' final days, weeks, and months as comfortable as possible.
Responsibilities
- Conduct initial client assessments and continuously refine care plans.
- Provide hands‑on nursing care and patient education.
- Maintain open lines of communication with patients, families, and the healthcare team.
- Manage patient care coordination for your caseload.
- Participate in interdisciplinary team meetings to ensure comprehensive care planning.
Qualifications
- Licensed as a Registered Nurse / RN in the state in which you work.
- Proficiency with Electronic Medical Records (EMR) software.
- Excellent planning, organizing, and communication skills.
- Ability to solve problems autonomously, ensuring responsibilities are met while maintaining high standards.
- Valid driver’s license and reliable vehicle with current insurance.
