• Assess patient conditions and develop tailored care plans.
• Maintain detailed medical records and communicate patient status.
• Serve as the primary liaison among patients, families, and healthcare providers.

• Coordinate with healthcare professionals to ensure continuous care.

Requirements:

• Active RN license in Arizona with no restrictions.
• At least 1 year of experience in in-home healthcare.
• Proficient in case management software, with strong organizational and communication skills.
• Empathetic, detail-oriented, and autonomous in field settings.

Compensation & Benefits:

• Salary: $82,500 - $97,800 per year, depending on experience
• On-call stipends and mileage reimbursement
