The RN Case Manager in the Neuro ICU at Massachusetts General Hospital plays a crucial role in enhancing patient care services along the continuum of care. This position is ideal for compassionate and skilled RNs who are dedicated to improving patient experiences and outcomes.

What You'll Do

  • Collaborate with the healthcare team to enhance patient care services.
  • Evaluate, predict, and facilitate the trajectory of patient care.
  • Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
  • Continuously improve the patient’s experience while ensuring high-quality patient care.
  • Participate in the Case Management department's orientation and onboarding program.
  • Engage in the "Novice to Expert" program for professional development.
  • Utilize resources from case management specialists to support patient care.

Requirements

Education:

  • ADN required, BSN strongly preferred

Experience:

  • Minimum of 2 years Med/Surg experience required
  • RN Case Manager experience required
  • Acute hospital experience preferred

Licenses & Certifications:

  • Current RN License in Massachusetts
  • Case Manager certificate preferred
