What You'll Do
- Collaborate with the healthcare team to enhance patient care services.
- Evaluate, predict, and facilitate the trajectory of patient care.
- Employ a collaborative process of assessment, planning, implementation, care coordination, monitoring, advocacy, and evaluation.
- Continuously improve the patient’s experience while ensuring high-quality patient care.
- Participate in the Case Management department's orientation and onboarding program.
- Engage in the "Novice to Expert" program for professional development.
- Utilize resources from case management specialists to support patient care.
Requirements
Education:
- ADN required, BSN strongly preferred
Experience:
- Minimum of 2 years Med/Surg experience required
- RN Case Manager experience required
- Acute hospital experience preferred
Licenses & Certifications:
- Current RN License in Massachusetts
- Case Manager certificate preferred
