RN Case Manager I - Telephonic / Field - Northeast Kansas

Job Summary

RN Case Manager I – Telephonic / Field – Northeast Kansas (JR ). The role is field‑based with 10‑15% travel across Wyandotte, Johnson and Miami counties. Requires residence in Kansas and current RN license. Schedule: 8am – 5pm, Monday‑Friday.

Responsibilities

• Ensure member access to appropriate health services.
• Conduct assessments virtually and in person to identify needs and develop care plans.
• Implement care plans by facilitating authorizations/referrals within benefits structure.
• Coordinate internal and external resources to meet identified needs.
• Monitor and evaluate effectiveness of care plans, modifying as necessary.
• Interface with Medical Directors and Physician Advisors on treatment plans.
• Negotiate reimbursement rates as applicable.
• Assist in problem solving with providers, claims or service issues.

Qualifications

• BA/BS in a health‑related field and minimum 3 years of clinical experience; or equivalent combination of education and experience.
• Current, unrestricted RN license in Kansas.
• Computer proficiency strongly preferred.

Preferred Skills, Capabilities and Experiences

• Strong communication and coordination skills.
• Experience with Medicaid or complex chronic care management.
• Ability to travel within Northeast Kansas.

Eligibility

Must reside in the state of Kansas; travel 10‑15% within Northeast Kansas. Not eligible for employment-based sponsorship.
