RN Case Manager I

We are seeking a Registered Nurse Case Manager to provide comprehensive care for members with complex needs, ensuring quality and cost-effective treatment.

Key Responsibilities Include

• Developing, implementing, and evaluating individualized care plans aligned with physician orders.
• Delivering competent, quality patient care in the home setting.
• Managing an assigned caseload and ensuring ongoing assessment and updates to care plans.
• Communicating effectively with staff, providers, and members' families.
• Providing patient education and participating in team knowledge sharing activities.
• Performing additional duties as assigned.

Qualifications And Skills

• Licensed RN in the Commonwealth of MA, with a minimum of 3-5 years' clinical home care experience.
• Bachelor of Science in Nursing preferred.
• Proficient in Microsoft Office and case management data systems.
• Ability to travel locally frequently.
• Spanish speaking and COVID vaccination preferred.
