Friendship Village seeks an RN Case Manager, Hospice to coordinate compassionate end-of-life care for seniors in our faith-based, mission-driven communities. In this role you will manage a caseload of hospice patients, complete comprehensive assessments, develop and update individualized care plans, and provide expert symptom and pain management. You will collaborate closely with physicians, social workers, chaplains, and therapists, while supporting and educating families.
