Responsibilities
- Complete initial and ongoing comprehensive assessments to determine hospice needs.
- Develop individualized care plans with the patient and family, updating as needs evolve.
- Administer medications and treatments as prescribed.
- Educate patients and caregivers on care techniques, medications, diet, and symptom management.
- Rotate on‑call duties and ensure seamless care transitions.
- Lead meaningful end‑of‑life conversations with patients and families.
Qualifications
- Associate's, Bachelor's, or Master's degree in an accredited nursing program (RN).
- 1+ years of recent acute care experience (hospice or home‑health experience preferred).
- Valid driver's license and reliable transportation with proof of auto insurance.

The starting salary range for this role is $78,000 to $97,000. The total compensation package may also include benefits such as medical/dental/vision/pet insurance, disability and life insurance, paid time off, and a 401(k) retirement plan.
