Responsibilities
- Conduct intake assessments with patients and families.
- Develop individualized care plans considering physical, financial, and other constraints.
- Coordinate patient care with physicians, nurses, and other healthcare professionals.
- Monitor patient progress and adjust treatment plans as needed.
- Ensure patients have access to necessary healthcare services and resources.
- Educate patients and their families about managing illnesses and treatment plans.
- Advocate for patients\' needs with insurance providers and healthcare facilities.
Qualifications
- Certification: Licensed Registered Nurse with no restrictions.
- Education: Bachelor\'s degree in nursing required; advanced degree preferred.
- Experience: At least 1 year as an RN Case Manager or similar role in hospice care.
- Skills: Includes strong interpersonal and communication skills; ability to work independently and as part of a team; proficiency in patient care documentation and management.
Compensation
- Pay: $85,000 - $100,000 annually
