POSITION SUMMARY

RN Case Manager – Hospice

POSITION DUTIES

Provides skilled nursing care, supportive care, and pain and symptom management to maximize comfort and quality of life for Hospice patients and their families. Provides care to patients in both homes and facilities.

MINIMUM REQUIRED QUALIFICATIONS

  • MA RN license
  • 1 year of nursing experience or 6 months of nursing experience with a BSN

PREFERRED QUALIFICATIONS

  • BSN
  • Hospice Home Care experience, IV Certification, Computer skills

LENGTH OF ASSIGNMENT

13 weeks

SHIFT / HOURS PER WEEK

8am-4pm, - weekday, weekend, holiday rotation
