RN Case Manager - Hospice Care Team Leader

St. Croix Hospice is seeking a Registered Nurse Case Manager to lead the patient’s hospice care team and develop the Plan of Care in collaboration with the Hospice Medical Director, patient, family, and facility staff.

The RN Case Manager provides day-to-day patient care, manages treatments, maintains records, and directs the care team to meet changing needs, focusing on comfort and quality of life.
