Moments Hospice is seeking an RN Case Manager on the road to provide compassionate, high-quality in-home care. You will perform initial assessments, refine care plans, and coordinate with families and the clinical team to ensure patient comfort during their final days, weeks, and months.
This field-based role includes a company car, gas and insurance coverage, and a robust benefits package. Competitive salary ranges from 75k–90k USD annually, with PTO and 401(k) options.
