What You'll Do
- Conduct comprehensive, ongoing assessments to understand each patient's evolving needs.
- Build and adjust personalized care plans in collaboration with physicians and care teams.
- Administer medications and treatments per physician orders.
- Keep clinical documentation accurate, timely, and meaningful.
- Serve as the communication bridge between families, physicians, and community health partners.
- Educate patients and caregivers so they feel confident and supported.
- Participate in on‑call rotation per team policy.

Requirements
- Active RN license in the state of employment.
- Graduate of an accredited school of Nursing.
- One (1)+ year hospice, home health or related organization experience preferred.
- Compassion for patients and families navigating end‑of‑life care.
- Strong clinical judgment and the ability to work independently in the field.
- Excellent communication skills across care teams and families.
