RN Case Manager — Home Health (Sign-On $10K)

Good Samaritan Society in Robbinsdale, MN is seeking an experienced RN Case Manager for home health to coordinate comprehensive patient care across pre-admission to post-discharge. You’ll lead the plan of care, collaborate with physicians and therapists, and ensure effective communication among the care team to optimize outcomes.

You will manage a diverse caseload, apply clinical judgment in field settings, and remain focused on safety, independence and quality care for patients in the community.
